On February 24, 2022, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; making statements that Licensee knew, or with the exercise of reasonable care should know, are false and misleading regarding efficacy or value of the medicine, treatment or remedy prescribed or administered by Licensee; gross and repeated acts of negligence; and willfully violating any board rule or order. This Order reprimands Licensee; assesses a $2,500 civil penalty; prohibits Licensee from treating male patients with any hormone except insulin; requires Licensee to perform at least five Level II or Level III office-based surgeries with a co-proceduralist before performing Level II or Level III office-based surgeries as a the sole proceduralist; subjects Licensee's practice to no-notice audits for two years; and requires Licensee to meet the following conditions when treating female patients with testosterone: document a diagnosis of Hypoactive Sexual Desire Disorder, obtain baseline free testosterone level, maintain testosterone level at pre-menopausal levels or lower, monitor testosterone levels every six months, and obtain patient's informed consent for treatment on a form pre-approved by the Board's Medical Director.

On July 12, 2018, Licensee entered into an Interim Stipulated Order to voluntarily follow the Endocrine Society Clinical Practice Guidelines in the treatment of patients with estrogen, testosterone, or thyroid hormones; cease the treatment of premenopausal women with testosterone; cease the treatment of male patients with testosterone whose testosterone levels are within the normal range; and cease the treatment of patients whose thyroid-stimulating levels are within the normal range pending the completion of the Board's investigation into her ability to safely and competently practice medicine.

On July 12, 2018, the Board issued an Order Terminating Order of Emergency Suspension. This Order terminates the Order of Emergency Suspension issued to License on June 7, 2018.
07/03/2018 07/12/2018 Order Of Emergency Suspension
On July 3, 2018, a Temporary Restraining Order (TRO) issued by the Marion County Circuit Court was rescinded subsequent to a hearing to show cause. The TRO held the Board's June 12, 2018, Order of Emergency Suspension in abeyance. The Order of Emergency Suspension suspends Licensee's medical license due to the Board's concern for the safety and welfare of Licensee's current and future patients. The suspension was effective from 5:00 pm June 13, 2018, to June 26, 2018, and is now effective from July 3, 2018, at 4:30 p.m., until otherwise ordered by the Board.
06/07/2018 06/26/2018 Order Of Emergency Suspension
On June 7, 2018, the Board issued an Order of Emergency Suspension to suspend Licensee's medical license effective June 13, 2018, due to the Board's concern for the safety and welfare of Licensee's current and future patients. This Order is in effect until otherwise ordered by the Board. On June 26, 2018, this Order was rescinded with a Temporary Restraining Order (TRO) by the Marion County Circuit Court. On June 27, 2018, the Marion County Circuit Court TRO was amended to reflect that the Order of Emergency Suspension was held in abeyance.
01/18/2018 02/24/2022 Interim Stipulated Order
On January 18, 2018, Licensee entered into an Interim Stipulated Order to voluntarily cease performing lipoplasty pending the completion of the Board's investigation into her ability to safely and competently perform this procedure.
07/13/2017 02/24/2022 Stipulated Order
On July 13, 2017, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct and gross or repeated acts of negligence. This Order assesses a $4,000 civil penalty, requires Licensee to complete courses on medical documentation and post-operative complication management; requires Licensee or a scheduled on-call physician to be available to post-operative patients within 30 minutes; requires Licensee obtain a practice mentor who will submit quarterly reports to the Board; requires Licensee to submit all post-operative complications to the practice mentor within 60 days; and subjects Licensee's medical records to no-notice audits by the Board.
