On May 5, 2022, Licensee entered into a non-disciplinary Corrective Action Agreement with the Board. In this Agreement, Licensee agreed to obtain pre-approved physician mentors in the areas of hospice medical care and death with dignity; to observe all physician steps of the Death with Dignity Act with ten patients prior to providing patient care pursuant to the Act; and to complete 50 hours of pre-approved end-of-life care CME.

On August 27, 2018, Licensee entered into an Interim Stipulated Order to voluntarily cease prescribing any medication for patients enrolled in hospice or requesting Death with Dignity, pending the completion of the Board's investigation.

On October 6, 2016, Licensee entered into a Stipulated Order with the Board for unprofessional or dishonorable conduct; gross or repeated negligence in the practice of medicine; willfully violating any provision of the Board's statute, rule, board order or board request; and prescribing controlled substances without a legitimate medical purpose, or prescribing controlled substances without following accepted procedures for examination of patients, or prescribing controlled substances without following accepted procedures for record keeping. This Order reprimands Licensee; assesses a civil penalty of $8,500; places Licensee on probation; requires Licensee to complete a CPEP education plan; requires Licensee to complete courses on medical documentation and prescribing; requires practice setting approval by the Board's Medical Director; subjects Licensee's practice to no-notice chart audits by the Board; requires Licensee to follow protocols when prescribing testosterone to male patients or antibiotics to any patient; and requires that Licensee only order lab tests which are supported in the medical record.
